Report a concern

For a possible breach of the code of ethics by someone or something the institute certifies. Named or anonymous — the committee reads both. You leave with a protocol number.

Optional. The committee identifies the certified party; you need only say what you know.

Leave both empty to report anonymously. With an email, the committee can ask you a question; it never shares your identity with the reported party.
